Hospice Services


Who is eligible?

Hospice of Kodiak provides end-of-life support services to clients/families experiencing a terminal illness of all ages. Any person in the Kodiak community who has a life-threatening diagnosis and a life expectancy of one year or less is eligible for our services, regardless of physical condition, age, race, religion or cultural affiliation.


How do patients get referred?

Hospice of Kodiak accepts referrals for Hospice services from medical providers, clients/families/caregivers, Senior Citizens of Kodiak and PKIMC medical staff. Referrals can be made by telephone or email.


How do Hospice of Kodiak services work?

A staff member will call and arrange a time to visit the client’s home to assess the client/family needs and our ability to help meet those needs. We may also make referrals to other agencies in the community that might better meet specific needs.

The volunteer coordinator will then match the client/family with a volunteer(s) with the appropriate skills.

The volunteer will make contact with the client/family and arrange a time to meet.
